IMPORTANT CONDITIONS

• You must register for a consultation or check up before 12/31.
• The offer is valid only for new clients (a new client is one who has never visited the clinic).
• The offer is valid for customers aged 18 years and over.
• To undergo an x-ray, you must have a referral from your attending physician. This can be a doctor from any clinic or a doctor from the Healthy & Happy clinic for an additional fee.
• Before purchasing a certificate, please contact the clinic by phone: +38 (044) 503-02-03 and check the availability of places for the desired date and time.
• The clinic operates under a license to carry out medical activities in accordance with the decision of the Ministry of Health of Ukraine.

ADDITIONAL TERMS

• Discounts on certificates cannot be combined with other promotional offers.
• To receive a discount, you must present a printed certificate or send the certificate to email info@hh.com.ua
• Please provide the last digit of the certificate (the activation code) only when receiving the service. Once activated, the service is considered provided, and the certificate is non-refundable.
• If you make an appointment but fail to show up at the appointed time and do not provide 24-hour notice of cancellation, or are more than 10 minutes late, the administration reserves the right to refuse discounted services, and the appointment is considered cancelled.
