IMPORTANT TERMS

• The offer is valid for clients over 18 years of age.
• One client can personally use only one certificate.
• Before purchasing a certificate, be sure to check availability for the desired date and time.
• When making an appointment, you must inform the clinic administrator about the availability of the certificate and present it at the start of the appointment.
• The medical center operates under a license to carry out medical activities in accordance with the decision of the Ministry of Health of Ukraine.

ADDITIONAL TERMS

• Certificate discounts cannot be combined with other promotional offers.
• To receive the discount, you must present the certificate in electronic form before the start of the appointment.
• State the last digit of the certificate (activation code) only when receiving the service. Once activated, the service is considered provided and the certificate cannot be returned.
• If you signed up for an examination, but did not show up at the specified time and did not warn about canceling the visit 12 hours in advance, and were also more than 30 minutes late, the administration reserves the right to refuse to provide services at a discount, and the examination is considered completed.
