• One certificate for the indicated amount can be used for several visits.
• The nominal value of the certificate is valid for all services, except for the MRI procedure.
• First, be sure to exchange the Coupon/Superdeal Certificate for a corresponding certificate from the company Proxy Kyiv at the number +38 (096) 812-83-85 and only after that contact the Eurospine center to register and receive the service.
• You can pick up a gift certificate yourself at the company's office or order delivery.
• It is possible to combine certificates.

ADDITIONAL TERMS

• One client can personally use an unlimited number of certificates.
• Certificate discounts cannot be combined with other promotional offers.
• To receive a gift certificate, you must present a discount certificate in electronic form (enter the discount certificate code on the website when picking up from the office or send the certificate code to the messenger, at the number specified in the contacts, when ordering delivery).
• Call the last digit of the certificate (activation code) only when receiving the service. After activation, the service is considered provided, and the certificate is non-refundable.
